Frontier Health consumers have broad access to a system of high quality behavioral health, mental health, substance abuse, co-occurring disorders, developmental disabilities, and vocational rehabilitation services in an integrated system of settings, services, health and behavioral health care practitioners and care levels.

   The continuum operates from pre-discharge screening through discharge of the patient providing assessment of on-going needs and coordinating care within Frontier Health’s scope of services and with agents, organizations and institutions external to Frontier Health.

   Coordination of care maximizes a consumer’s access to appropriate care, choice of available care, transition through levels of care and effective referral and discharge planning and communication.
