  Continuous Treatment Team (CTT) is an intensive therapeutic in-home program offering both therapy and case management services.

   It is designed for families whose children are at-risk of hospitalization, out-of-home placement, or are being transitioned back into the home from placement.
    CTT serves consumers in the eight Northeast Tennessee counties serviced by Frontier Health.

CTT Services
    Continuous Treatment Team works with families to design services to fit the unique needs of each family.

   Following assessment of the child, a professional team of behavioral health specialists provide:


 • In-home family therapy.
 • Linkage to Frontier Health services.
 • Intensive case management.
 • Crisis intervention.
 • Parenting assistance.
 • Individualized behavioral plans.
 • Linkage to educational services and other

   agencies.

Assisting Children
    These services are designed to assist children maintain progress during psychiatric hospitalization or to assist children remain at home as an alternative to psychiatric hospitalization.

Criteria for Admission to include:
 • DSM diagnosis of major mental illness.
 • Evidence of serious emotional disturbance as
    indicated by a TPG of 2 or lower.
 • History of hospitalization in an acute
    psychiatric setting or other out-of-home
    placement.
 • History of being hospitalized in an acute
    psychiatric setting two or more times in less
    than a year.
 • High risk of potential acute hospitalization.
 • Dual diagnosis with substance abuse.
 • Multiple contacts with crisis response teams.
 • History of homelessness.
 • History of involvement with the Juvenile
    Justice system.
 • A major weakening of the family’s support
    system.
 • Inability to respond/comply with traditional
    treatment and intensive case management.
 • Limited understanding/acceptance of mental
    illness and tools to manage illness.
